PA License Plate Campaign
Order an Autism Society of America custom licence plate. These plates serve as a public symbol of support for the autism community.
How to apply for a Special Registration Plate:
1. Download form MV-904SP (7-25) 2. Make sure name of organization in Section B is Autism Society of America (P5)3. For a Standard Plate fill out section “A” and section “E”. 4. The cost is $59. (This includes a PennDOT fee of $34 plus the organization fee of $25 to Autism Society Greater Philadelphia). 5. For a Specialty “Vanity” Plate fill out sections “A”, “D” and “E”. 6. The cost is $195. (This includes the specialty plate fee of $136, the PennDOT Fee of $34 and the $25 organization fee to Autism Society Greater Philadelphia)**If you already have a vanity plate you still need to pay the specialty plate fee again to keep your special set of letters and/or numbers. 7.
